Guided bone regeneration in fractured molar tooth: case report

Objective: To report a clinical case of guided bone regeneration in a fractured molar with chronic abscess, highlighting its relevance for successful implant rehabilitation. Case Report: A 48-year-old male patient (S.C.S.) was referred by an endodontist to a private clinic with a complaint of tooth fracture in the mandibular left first molar (tooth 36) and a history of chronic vestibular abscess. After unsuccessful endodontic treatment and identification of a crown-root fracture, tooth extraction with guided bone regeneration and possible immediate implant placement was indicated. Extraction was performed preserving the interradicular septum, followed by immediate implant placement with a torque of 40 N, filling of the socket with particulate xenogeneic graft (Geistlich Bio-Oss), covered with a resorbable membrane (Techgraft – Baumer) and a non-resorbable membrane (Lumina BOPP – Critéria). The non-resorbable membrane was removed after 21 days, and at 180 days, a new CT scan showed regeneration of the buccal bone plate and complete osseointegration of the implant. Conclusion: Guided bone regeneration combined with immediate implant placement proved to be effective in preserving the alveolar ridge and achieving primary stability, representing a predictable and safe approach in areas previously affected by chronic abscess.
