Abstract
Introduction: immediate implant placement after tooth extraction, combined with bone grafting and a customized healing abutment, aims to reduce treatment time, preserve bone and gingival contours, and optimize esthetics and function. Tooth loss, especially within the first three months, leads to significant anatomical changes in the alveolar ridge and soft tissues. Objectives: to report a clinical case of immediate implant placement in the posterior maxilla with bone grafting and a customized healing abutment; to assess tissue preservation, emergence profile, and peri-implant esthetics; to compare outcomes with the literature; and to contribute to safe and predictable clinical protocols. Methodology: a systematic literature review was conducted in SciELO, BDTD, and PubMed, including publications from the last ten years, with 14 articles selected. A case report of a 41-year-old patient with an extensive fracture of tooth 16 was presented. Treatment included atraumatic extraction, placement of a Cone Morse implant with 45 N torque, filling with synthetic bone graft and collagen membranes, and a customized healing abutment made of flow resin. The patient was followed up for six months with clinical and radiographic evaluations, digital rehabilitation, and prosthetic installation. Conclusion: the combination of immediate implant placement, bone grafting, and a customized healing abutment promoted tissue preservation, maintenance of the emergence profile, esthetic and functional stability, and was well accepted by the patient. The case confirms the literature and reinforces the importance of careful planning and strict protocols in Implant Dentistry.
