Introduction: Down syndrome (DS) is a genetic condition that can lead to several oral alterations, such as tooth agenesis, malocclusion, and periodontal involvement. The dental approach in these patients requires multidisciplinary attention, individualized planning, and clinical sensitivity. Objective: To report the clinical case of a patient with DS who underwent prosthetic rehabilitation, highlighting the challenges and strategies adopted to restore function, aesthetics, and oral health. Case Report: A 37-year-old female patient, residing in Serra, Espírito Santo, was referred by APAE (Brazilian Health Agency) to the Multivix Vitória dental clinic with a chief complaint of gingival lesion in the region of tooth number 12 and 22. She used an unsatisfactory removable temporary prosthesis (RPP), in addition to having deficient restorations, malocclusion, conoid teeth, retained deciduous teeth, and compromised oral hygiene. Clinical and radiographic examination revealed multiple missing teeth, carious lesions, and inadequate endodontic treatment. The therapeutic plan began with periodontal scaling and laser therapy to control gingival inflammation. After adapting the oral environment, study impressions were taken and analyzed jointly by a specialized team. Carious lesions were removed and composite resin restorations were performed on teeth 11 and 21. The rehabilitation phase involved replacing the RPD with a tooth-supported fixed prosthesis on teeth 13, 11, 21, and 23, with prosthetic preparations adapted to the anatomy of the abutment teeth. Accurate impressions and color selection were performed according to clinical protocol. Conclusion: Prosthetic rehabilitation in patients with DS requires a careful and interdisciplinary approach. This case demonstrated that, with adequate planning and attention to anatomical and behavioral particularities, satisfactory functional and aesthetic results can be achieved, promoting health and quality of life for the patient.
