Aumento de coroa clínica estético como etapa prévia a facetas diretas: relato de caso clínico

Objective: To report a clinical case of periodontal plastic surgery for clinical crown lengthening in the esthetic zone, as a preparatory step for rehabilitation with direct composite resin veneers. Case report: A 26-year-old male patient sought dental care with complaints of persistent gingival inflammation and compromised esthetics. Clinical examination revealed necrotizing gingivitis associated with the presence of dysbiotic biofilm and inversion of the interdental papillae between the maxillary incisors and canines. Oral hygiene instructions and antimicrobial photodynamic therapy (aPDT) were performed in two sessions, with an eight-day interval. After remission of gingival inflammation, digital planning was carried out based on clinical photographs and measurement of tooth width and height, using the golden proportion (80%) to define the new ideal height of the clinical crowns. Thus, the level of the bone crest and the amount of osteotomy required were determined. The surgical procedure involved internal bevel gingivectomy with a full-thickness flap and osteotomy of the alveolar bone crest, aiming for harmony between soft and hard tissues for subsequent esthetic rehabilitation. Results: After three months of clinical follow-up, satisfactory healing, stability of the gingival margin, absence of inflammation, and a significant improvement in smile esthetics were observed. Conclusion: Esthetic clinical crown lengthening, preceded by effective inflammation control through photodynamic therapy and guided by digital planning and golden proportion, proved to be effective and predictable as a preparatory step for esthetic rehabilitation with direct composite resin veneers, ensuring tissue stability and adequate support for the restorative outcome.
