USO DE L-PRF (FIBRINA RICA EM PLAQUETAS E LEUCÓCITOS) EM CIRURGIA ORAL

Abstract

Tissue regeneration remains a constant challenge in oral surgery, particularly in procedures involving bone and soft tissue loss. In this context, leukocyte- and platelet-rich fibrin (L-PRF) has emerged as an autologous biomaterial capable of modulating inflammation and promoting tissue healing. This study is a narrative literature review aiming to gather and analyze the available evidence on the use of L-PRF in oral surgery, highlighting its clinical applications, benefits, and limitations. The literature search was conducted in PubMed, SciELO, ScienceDirect, and Google Scholar, considering publications from 2018 to 2025. Original articles, reviews, and case reports addressing L-PRF applications in oral surgical procedures were included. The analyzed studies demonstrated that L-PRF provides significant clinical advantages, particularly in reducing postoperative pain, edema, and trismus, while accelerating tissue repair. In third molar extractions, alveolar grafting, and sinus lift procedures, improved inflammatory parameters and bone regeneration were observed. Case reports also highlighted successful outcomes in complex situations, such as oronasal fistula treatment and bone reconstructions. Despite these promising results, some studies pointed out that L-PRF efficacy depends on preparation protocols, surgical techniques, and individual patient characteristics, with limited differences compared to conventional treatments in long-term follow-ups. In conclusion, L-PRF appears as an effective and low-cost biological adjuvant to enhance tissue repair in various oral surgery procedures. However, further controlled clinical trials are necessary to standardize its use and confirm its long-term effectiveness.
