TRATAMENTO CIRÚRGICO DE SIALOLITÍASE EM GLÂNDULA SUBMANDIBULAR: RELATO DE DOIS CASOS.

Abstract

Sialolithiasis is a condition characterized by obstruction of a salivary gland. This obstruction occurs due to the presence of sialoliths (calcified structure that develops within the duct system of the salivary glands). The formation of sialoliths can be caused by chronic sialodenitis and partial obstruction. The submandibular gland is the most affected (80%). The most common signs and symptoms are pain and local swelling, especially when there is salivary stimulation. The objective of this work is to report the management and treatment of removal of a submandibular gland affected by sialolithiasis in two patients. Patients, aged 52 and 28, both male, attended the Hospital Santo Antônio (OSID), in the city of Salvador – BA, complaining of pain and increased submandibular volume. Imaging examinations revealed the presence of a radiopaque lesion on the panoramic radiograph of the first patient and a hypertensive lesion on the facial tomography of the second patient, suggestive of sialoliths present in the ductal system of the submandibular gland. Based on these findings, the diagnosis of sialolithiasis was concluded and the patients underwent surgical removal of the affected submandibular glands. Extraoral access was performed, in the submandibular region, according to the affected sides, exposing the submandibular gland, followed by its removal. Post-operatively, lasting approximately more than a week, the two patients had no clinical repercussions and had satisfactory tissue healing. Therefore, the choice of treatment for Sialolithiasis should be based on the size, location and gland that was affected. Thus, surgical treatment to remove the salivary gland is an effective method and should always be taken into consideration in the face of conditions that indicate it, as in the cases discussed.
