Ação Anti-inflamatória e Analgésica do Canabidiol no Tratamento de Feridas Neuropáticas e Isquêmicas Complexas

Resumo

As feridas neuropáticas e isquêmicas complexas representam um expressivo desafio clínico devido ao microambiente de inflamação crônica, elevado estresse oxidativo e dor de difícil controle. O objetivo deste estudo foi analisar as evidências científicas sobre as ações anti-inflamatórias, analgésicas e regenerativas do canabidiol (CBD) e outros derivados da Cannabis sativa no tratamento dessas lesões. Trata-se de uma revisão integrativa da literatura desenvolvida nas bases PubMed, SciELO, BVS, Web of Science e Scopus, guiada pela estratégia PICo. A amostragem final foi constituída por 16 estudos científicos. Os resultados apontam que o CBD atua na modulação do Sistema Endocanabinoide e de receptores periféricos, inibindo citocinas pró-inflamatórias, bloqueando a síntese de prostaglandinas e reduzindo a hiperalgesia periférica sem gerar efeitos psicoativos centrais. A combinação do fitocanabinoide com sistemas biotecnológicos de liberação, como hidrogéis inteligentes e matrizes transdérmicas, favorece a remoção de espécies reativas de oxigênio, estimula a neovascularização e garante a entrega sustentada do fármaco no leito da lesão. Conclui-se que o CBD se consolida como uma alternativa terapêutica promissora e integrativa, capaz de reverter a cronicidade das úlceras complexas, otimizar a cicatrização e proporcionar alívio analgésico eficaz.
