Persistência tecidual e complicações tardias associadas a preenchedores à base de ácido hialurônico: uma revisão integrativa

Introdução: Os preenchedores à base de ácido hialurônico (AH) são amplamente utilizados em procedimentos estéticos e, embora sejam considerados biodegradáveis, evidências recentes têm demonstrado que o material pode permanecer nos tecidos por períodos superiores aos tradicionalmente esperados, levantando questões acerca de seu comportamento e de possíveis complicações em longo prazo. Objetivo: Analisar as evidências disponíveis acerca da persistência prolongada e da presença de resíduos teciduais de preenchedores à base de AH, bem como das reações granulomatosas e demais manifestações inflamatórias tardias associadas ao seu uso. Metodologia: Trata-se de uma revisão integrativa da literatura, de caráter descritivo. A busca foi realizada em janeiro de 2026 nas bases PubMed, MEDLINE e LILACS, contemplando estudos publicados entre janeiro de 2022 e janeiro de 2026. Foram identificados 407 registros e, após a remoção de duplicatas, triagem por títulos e resumos e avaliação dos textos completos segundo os critérios de elegibilidade, 23 estudos constituíram a amostra final. Os achados foram analisados de forma descritiva e comparativa e organizados em três eixos temáticos. Resultados e Discussão: Os estudos demonstraram que preenchedores de AH podem permanecer detectáveis nos tecidos por períodos superiores a dois anos e, em determinados casos, por mais de uma década. As reações granulomatosas apresentaram ampla variabilidade temporal e diferentes padrões histopatológicos, incluindo granulomas de corpo estranho associados ao material de preenchimento. As manifestações inflamatórias tardias compreenderam principalmente edema, nódulos e reações de hipersensibilidade, com relatos temporalmente associados à infecção ou vacinação contra SARS-CoV-2, além de outros possíveis eventos desencadeantes. Considerações finais: As evidências indicam que a permanência tecidual dos preenchedores de AH pode ser substancialmente mais prolongada do que tradicionalmente considerada e que as complicações tardias apresentam natureza heterogênea e multifatorial. Entretanto, a presença de material residual não implica, isoladamente, condição patológica, e as evidências disponíveis ainda são insuficientes para estabelecer a ocorrência e a relevância clínica de um possível efeito cumulativo decorrente de aplicações sucessivas. Estudos prospectivos e longitudinais são necessários para esclarecer a dinâmica de degradação, a persistência após reaplicações e sua eventual relação com complicações tardias.
