Acquired Syphilis in Minas Gerais: An epidemiological analysis from 2010 to 2024 challenges and future prospects

Introduction: Acquired syphilis is transmitted by the bacterium Treponema pallidum through sexual or vertical contact. In 2022, global cases rose to 8 million, with the highest incidence in the Americas, following the COVID-19 pandemic. Objective: To conduct a descriptive epidemiological analysis of syphilis in the state of Minas Gerais between 2010 and 2024. Methods: The study was based on data from the Notifiable Diseases Information System (SINAN) for the period from 2010 to 2024, examining the variables of age group, sex, race, distribution by macroregions, diagnostic criteria, and case progression. Results: From 2010 to 2024, 136,788 cases of acquired syphilis were reported in Minas Gerais, with a cumulative incidence of 666 cases per 100,000 inhabitants and a case fatality rate of 0.057%; males (63.7%), people of mixed race (45.7%), and those aged 20 to 39 years were overrepresented. The distribution showed a seasonal pattern, with the highest number of reported cases between July and November, and the central macroregion accounting for the largest proportion of reported cases (47.3%). Discussion: This increase in acquired syphilis in Minas Gerais is consistent with the national trend and may be explained by risky sexual behaviors, diagnostic failures, or expanded testing through epidemiological surveillance. The decline in 2020 may be related to the COVID-19 pandemic due to reduced demand for health services. Meanwhile, the decrease between 2023 and 2024 likely reflects the incompleteness of recent data. These findings may underscore the need for integrated strategies to address the problem, including active screening of high-risk populations, expanded access to diagnosis, timely treatment of cases, and educational initiatives aimed at promoting condom use and reducing risky sexual behaviors. Conclusion: The increase in cases of acquired syphilis in Minas Gerais reflects the national and global trend of the reemergence of the infection, associated with social determinants such as sex, race, and educational level, highlighting the need to strengthen integrated public policies for prevention, diagnosis, and epidemiological surveillance.
