Vertical and horizontal reconstruction of the premaxilla using the Khoury technique: a case report

Abstract

Successful implant-supported rehabilitation requires adequate bone volume for proper implant positioning and predictable esthetic outcomes. In cases involving bone loss associated with poorly positioned implants, reconstructive procedures may be necessary before a new rehabilitation. The Khoury Technique has been widely used for horizontal and vertical bone augmentation through the use of autogenous cortical plates associated with particulate graft materials. This study reports the removal of previously installed implants in the anterior maxilla and immediate bone reconstruction using the Khoury Technique associated with PRF Bone Graft.

A 64-year-old male patient presented with a four-unit fixed prosthesis supported by two implants in the maxillary central incisor region and reported esthetic dissatisfaction, seeking rehabilitation with individual crowns. Clinical and tomographic evaluation revealed unfavorable implant positioning and local bone deficiency. Implant removal and immediate reconstruction were performed using two autogenous cortical plates harvested from the mandibular retromolar region associated with PRF Bone Graft. The patient remains under postoperative follow-up for graft incorporation and future implant rehabilitation planning.
