Late Diagnosis and Delay in Treatment of Cervical Cancer in the Brazilian Amazon: Evidence from a Reference Hospital

Abstract

Introduction: Cervical cancer (CCU) is the second most common neoplasm in the North and Northeast regions of Brazil and is responsible for 7.5% of all cancer deaths in the female population. Objective: To identify the stage at diagnosis and the interval between diagnosis and the start of treatment of cervical cancer in women treated at a reference hospital in the Brazilian Amazon. Methodology: Retrospective study, conducted with data from the Hospital Cancer Registry of a public hospital in Roraima, from 2018 to 2023. Sociodemographic variables, clinical staging, and the interval between diagnosis and treatment were analyzed using descriptive statistics. Results: 271 women were included, with a mean age of 46.1 years. A predominance of diagnoses in advanced stages was observed, with stages IIB (29.15%) and IIIB (28.78%) standing out, totaling 57.93% of cases. Early stages represented 19.19%. The average time between diagnosis and the start of treatment was 157 days, with a median of 142 days, exceeding the legal deadline of 60 days. Conclusion: A high frequency of late diagnosis and delays in starting treatment were evident, indicating weaknesses in screening and in the organization of the healthcare network. This reinforces the need to strengthen early detection efforts and ensure timely access to treatment, especially in more vulnerable regions.lth.

Currently, he is the Director of Sanitary Surveillance at the Municipal Health Department of Boa Vista

Patrícia Claro dos Santos Fuly, Universidade Federal Fluminense

She holds a degree in Nursing from the Federal Fluminense University (2000), a master's degree in Biomedical Engineering from the Federal University of Rio de Janeiro (2004), and a doctorate in Nursing from the Anna Nery School of Nursing at the Federal University of Rio de Janeiro (2009). She completed a postdoctoral fellowship at the USP School of Nursing (2014), funded by CNPq. Currently, she is a professor at the Aurora de Afonso Costa School of Nursing at the Federal Fluminense University (EEAAC/UFF), where she works as an Associate Professor IV in undergraduate courses, teaching Nursing in Adult and Elderly Health I, Studies in Oncology, and Introduction to Research Methodology. She is a permanent professor/supervisor of Master's and Doctoral students in the Postgraduate Program in Health Care Sciences (PACCS). Vice-coordinator of PACCS (2022-2026 term). Master's degree in Collective Health from the Postgraduate Program in Collective Health at the Federal University of Espírito Santo (PPGSC-UFES). Bachelor's degree in Nursing from the Federal University of Espírito Santo (UFES). Currently a professor in the Nursing course at the Federal University of Roraima. She is a member of the research groups "Women's, Children's and Adolescents' Health - Social determinants, epidemiology and evaluation of policies, programs and services." She was a scholarship monitor in the Education through Work for Health Program (PET Saúde) with an emphasis on the epidemiological profile of adolescents in the Greater Maruípe region - Vitória (ES); a Nursing intern at the Clinical Research Center of the Cassiano Antonio de Moraes University Hospital; a volunteer monitor for the Epidemiological Studies in Nursing course, and a volunteer in the Institutional Scientific Initiation Program (PIVIC) at the Federal University of Espírito Santo.

Graduated in Nursing and Obstetrics from the Fundação Educacional da Serra dos Órgãos (1994). Master's degree in Education from the Pontifícia Universidade Católica de Petrópolis (2000), PhD in Nursing from the Anna Nery School of Nursing at the Federal University of Rio de Janeiro (2003), Post-Doctorate in Nursing from the Federal University of Santa Maria - RS (2014) and Specialization in Gynecological and Obstetric Nursing (2014). Currently, he is a Researcher and Full Professor at the Federal Fluminense University in the maternal-infant area of ​​the Maternal-Infant and Psychiatric Department of the Aurora Afonso Costa School of Nursing/UFF and Professor of the Professional Master's Program in Maternal-Infant Health at the Faculty of Medicine - UFF. Professor of the Academic Master's and Doctorate in Health Sciences EEAAC/UFF. Experience in the field of Health and Nursing, with emphasis on Maternal and Child Health, mainly working on the following topics: Women's Health, Paternity, Prenatal Care / Childbirth and Birth / Postpartum and Reproductive Planning. Graduated in Nursing from the Federal University of Roraima (2018). Specialist in Nursing Teaching from FAVENI (2022). Master in Health Sciences from UFRR (2025). PROFESSIONAL: Currently Superintendent of Health and Environmental Surveillance in Boa Vista and Professor of the Nursing Course at Claretiano Faculdade Boa Vista and of preparatory courses for public service exams in the nursing field. He has previously worked as a Supporter of the Strategic Information Center for Health Surveillance in Boa Vista for the Department of Public Health Emergencies of the Ministry of Health. He has experience in the field of Health Surveillance: he was the coordinator of the Strategic Information Center for Health Surveillance in Boa Vista - RR (capital) 2021/2022, where he assisted in managing the COVID-19 Public Health Emergency through the preparation of Alerts, Risk Communications and daily Epidemiological Bulletins, and coordinated Situation Rooms for other health problems such as Measles, Arboviruses and Monkeypox. He defended his undergraduate thesis entitled "Proposal for systematizing nursing care for patients diagnosed with Chikungunya Fever treated in an Emergency Care Unit in Boa Vista - RR". He also defended his specialization thesis entitled: "The teaching-learning process in nursing: challenges encountered by teachers and students in the field of internship".
