The Physiotherapeutic Approach to Hill-Sachs Injury: A Literature Review

Abstract

Hill-Sachs lesions are frequently associated with glenohumeral instability and recurrent shoulder dislocations, being characterized by symptoms such as pain, functional limitation, crepitus, and a sensation of joint instability, especially during abduction and hyperextension movements. This study aimed to describe the physiotherapeutic approach to Hill-Sachs lesions. A literature review was conducted using the Scientific Electronic Library Online (SciELO), Latin American and Caribbean Health Sciences Literature (LILACS), and Virtual Health Library (VHL) databases, considering articles published between 2020 and 2026. The analyzed studies demonstrated that physiotherapy plays a fundamental role in both conservative and postoperative treatment, contributing to functional improvement, recovery of range of motion, muscle strengthening, scapular stabilization, and pain reduction. Furthermore, significant impact on the prevention of recurrence was observed, since patients with recurrent instability often present proprioceptive deficits, biomechanical alterations, and residual muscle weakness. The literature also demonstrated that therapeutic success does not depend exclusively on the surgical technique employed, but also on the quality of subsequent physiotherapeutic rehabilitation. It is concluded that physiotherapy plays an essential role in the management of Hill-Sachs lesions, acting directly on functional recovery, joint stability, and prevention of recurrent dislocations.
