Primeiro relato de caso de infecção da cavidade oral por Candida magnoliae em um paciente com câncer

Abstract

Introduction: Infections caused by non-albicans Candida species have increased globally, especially among immunosuppressed and hospitalized patients, with changes in the epidemiological profile of candidemia in Brazil.

Objective: To report the first worldwide case of isolation of Candida magnoliae in the oral cavity of a patient undergoing oncological treatment.

Case report: A 20-year-old melanoderma male patient undergoing radiotherapy for a previously resected cerebral astrocytoma presented with tongue coating, hyposalivation, and oral pruritus during a dental consultation. He reported frequent contact with birds. Samples from the tongue and buccal mucosa were collected using sterile swabs and cultured on Sabouraud dextrose agar with chloramphenicol. Identification and antifungal susceptibility testing were performed using the automated Vitek® system, with interpretation according to EUCAST/BrCAST guidelines. The patient was diagnosed with Candida magnoliae infection and treated with fluconazole, showing complete resolution of symptoms after therapy.

Conclusion: The isolation of Candida magnoliae in the oral cavity of an immunosuppressed patient represents an unprecedented finding, reinforcing the importance of microbiological surveillance and accurate identification of emerging fungal species. These microorganisms may have relevant clinical and therapeutic implications, expanding knowledge about the genus Candida and highlighting the need for further research on their pathogenicity and antifungal susceptibility profile.
