Lower premolar with two root canals and periapical bone radiolucency

A female patient, of mixed race, presented to the dental office complaining of pain upon touch in tooth 44. Periapical radiographic examination revealed the presence of two root canals due to abrupt narrowing in the cervical third, as well as a radiolucent area near the root apex. After anesthesia, the restoration was removed and the coronal opening was performed with a #1013 bur. Subsequently, the root canals were explored with manual files (#8, #10, or #15). After electronic foraminal odontometry, root canal preparation was performed with Solla Files Collors Reciprocating #R25 files, and foraminal patency was achieved with Solla Files Collors 16/02 files to the actual length of the tooth. Obturation of the root canal system was performed using the single cone R#25 technique associated with AH-Plus cement. It is concluded that it is important to alert oral health professionals about the knowledge of the root anatomy of lower premolars to ensure their success in endodontic treatment.
