Orthodontic Treatment in patients with Periodontal Disease

ABSTRACT
Orthodontic treatment in patients with periodontal disease represents a significant clinical challenge, since tooth movement occurs in a biological environment that may present bone support loss and inflammatory alterations. In these cases, treatment planning must consider periodontal stability, dental biofilm control, and the integration between different dental specialties, particularly Orthodontics and Periodontology. The aim of this study was to analyze the relationship between orthodontic treatment and periodontal disease, as well as to discuss the main clinical recommendations for managing patients with periodontal compromise undergoing orthodontic tooth movement. This study consists of a narrative literature review. The bibliographic search was conducted in the databases PubMed, LILACS and SciELO, with complementary searches performed in Google Scholar. Articles published between 2010 and 2024 in Portuguese and English that addressed the relationship between orthodontics and periodontal disease were considered. After screening titles and abstracts, the most relevant studies were selected for full-text analysis and evidence synthesis. The literature indicates that orthodontic treatment can be safely performed in patients with periodontal compromise, provided that periodontal disease is previously controlled and that continuous periodontal monitoring is maintained throughout treatment. Furthermore, the application of light orthodontic forces, strict oral hygiene control, and regular clinical follow-up are essential factors to reduce risks and ensure periodontal stability. In conclusion, orthodontic treatment may provide functional and aesthetic benefits when performed with careful planning and an interdisciplinary approach. However, therapeutic success depends on prior periodontal stability, patient cooperation, and appropriate clinical monitoring throughout all phases of treatment.
