Unicystic ameloblastoma is a benign but potentially recurrent odontogenic tumor commonly located in the mandible. Radical treatment often involves marginal or segmental resection; however, conservative strategies such as marsupialization have gained prominence due to their ability to preserve anatomical structures and reduce morbidity. Objective: To evaluate the effectiveness of marsupialization as a conservative approach for unicystic ameloblastoma by discussing indications, surgical technique, clinical outcomes, and recurrence rates. Methods: A literature review was conducted using articles indexed in Google Scholar, Scopus, and Pubmed selected through descriptors related to unicystic ameloblastoma and conservative treatment. Conclusion: Evidence suggests that marsupialization can lead to substantial lesion reduction and bone preservation when applied to appropriately selected cases. Long-term follow-up remains essential given the possibility of recurrence.
