Introduction: Gingival recession is characterized by apical migration of the gingival margin in relation to the cemento-enamel junction, with root exposure, which can cause a series of factors, such as: dental hypersensitivity, gingival inflammation and aesthetic discomfort. The literature reports that Subepithelial Connective Tissue is the gold standard therapy for the treatment of these alterations. A more current and promising approach in surgical cases is treatment with Advanced Platelet-Rich Fibrin (A-PRF) has been proposed as an adjuvant biomaterial that aims to enhance tissue healing and improve clinical outcomes. This study presents a randomized controlled study of multiple root covering surgeries with and without the use of A-PRF, seeking to evaluate its benefits. Objective: To analyze, through a report of a randomized study, the results of root coverage using connective tissue grafting, with and without the association of advanced platelet-rich fibrin, comparing healing, insertion gain, percentage of coverage and periodontal aesthetics. Methodology: This randomized controlled study obtained 6 sites with defects and were distributed into 2 groups, 3 treated with CTE alone (control group) and 3 treated with CTE + A-PRF (experimental group). Probing depth, gingival recession depth, and gingival thickness were evaluated clinically and by comparative tomographic analysis (T0- and T1-weighted CBCT) at T0 and T1 (90 days). Conclusion:Both techniques proved to be efficient, resulting in significant partial root coverage and transformation of the periodontal phenotype. Connective Tissue Grafting (CTG) was ratified as the structural gold standard for volumetric input and tissue stability, while A-PRF acted as a strategic biological additive, raising the standard of healing and patient comfort, although it did not present superiority in terms of structural support in the short term.
