Incidência e implicações cirúrgicas da inervação sensitiva pelo nervo milo-hióideo na mandíbula anterior

The presence of sensitivity during extractions of mandibular incisors, even after proper administration of an inferior alveolar nerve block, may be associated with accessory innervation from branches of the mylohyoid nerve. Anatomical and histological studies demonstrate that this nerve, traditionally described as a motor branch, may give rise to sensory fibers that reach the anterior teeth through small accessory foramina located on the lingual surface of the mandible.
Understanding this anatomical particularity is essential for adequate anesthetic planning, contributing to greater predictability, reducing the need for supplemental injections and increased doses of local anesthetics, and thereby enhancing patient comfort during surgical procedures, which inherently tend to generate apprehension.
In this context, the present study aims to review the literature regarding the incidence of this anatomical variation, discussing its clinical implications in anesthetic failures observed in surgical procedures involving the anterior region of the mandible.
