Prevention of Pressure Ulcer Injuries in Stroke Patients: Targeted Nursing Care.

Introduction: Pressure ulcer injuries (PUIs) are a common and preventable adverse event in stroke patients, associated with immobility, functional dependency, and insufficient continuity of care. Objective: To synthesize evidence indexed in PubMed/MEDLINE on nursing care aimed at preventing PUIs in stroke patients. Methodology: Systematic review following PRISMA 2020. Original studies involving adults with stroke and outcomes related to PUIs or risk, published between 2016 and 2025 and indexed in PubMed/MEDLINE, were included. Title/abstract and full-text screening, standardized data extraction, and narrative synthesis were performed due to the heterogeneity of designs and settings. Results: Eight studies were included (RCTs, quasi-experimental, cohort, cross-sectional, quality improvement). In hospitals, the reported prevalence of PUIs was 8.3%, and during transition to SNF, 11% were admitted with PUIs. A nurse-led program reduced the HAPI rate from 1.177 to 0.272 per 1,000 patient days; post-discharge, telenursing, and caregiver training decreased the incidence/severity or risk of PUIs. Conclusion: Effective prevention in stroke patients requires nursing bundles that include surveillance, repositioning, support surfaces, education, caregiver follow-up, and risk stratification to prioritize resources.
