SURGICAL MANAGEMENT FOR THE TREATMENT OF RECURRENT RADICULAR CYST: A CASE REPORT

Abstract

Introduction: Radicular cysts are inflammatory odontogenic lesions of epithelial origin, frequently associated with pulp necrosis and chronic infectious processes. Although endodontic treatment is the first therapeutic choice, recurrent cases may require surgical intervention to ensure complete resolution of the lesion. Correct management is essential to prevent complications, preserve anatomical structures, and restore masticatory function and aesthetics. Objective: To report a clinical case of a recurrent radicular cyst located between the maxillary lateral incisor and the first premolar, treated surgically, highlighting the technical aspects, postoperative evolution, and relevance of interdisciplinary management. Case Report: A 35-year-old male patient presented with increased volume in the anterior region of the maxilla, between the lateral incisor and the first premolar, associated with mild pain and a history of previous endodontic treatment of the lateral incisor. Radiographic examinations showed a radiolucent image compatible with a recurrent radicular cyst. Surgical enucleation of the lesion under local anesthesia was chosen, followed by apicoectomy. The specimen was sent for histopathological analysis, confirming the diagnosis of a radicular cyst. The postoperative period progressed without complications, with edema regression within five days and satisfactory healing. Radiographic follow-up after six months demonstrated adequate bone neoformation and absence of recurrence. Conclusion: Surgical management of the recurrent radicular cyst located between the lateral incisor and the first premolar proved effective in resolving the case. Surgical intervention is a safe and necessary alternative when endodontic treatment alone is insufficient. Correct indication, combined with precise technical execution and clinical and radiographic follow-up, are crucial for therapeutic success.
