Jugulotympanic Tumor Lesions: A Literature Review.

Abstract

Introduction: Jugulotympanic tumor lesions are hypervascular paragangliomas with slow growth; extension to the jugular foramen can compromise lower cranial nerves, turning the issue into an otoneurological problem. Objective: To synthesize evidence in PubMed on clinical presentation, diagnosis, and outcomes of surgery, radiosurgery, stereotactic radiotherapy, and surveillance. Methodology: A PRISMA 2020 systematic review with an exclusive PubMed search between 2020-2025. Results: The 24 studies included meta-analyses/systematic reviews, surgical series, radiosurgery series (Gamma Knife/LINAC), and transcanal endoscopic series for tympanic tumors. Overall, surgery and SRS achieved high local control; in aggregate synthesis, surgery showed greater morbidity (lower cranial nerves, dysphagia, CSF fistula). Transcanal endoscopy was effective for selected tympanic lesions. Preoperative embolization was associated with less bleeding and shorter surgical time, with heterogeneous evidence. Discussion: There is no universal approach; the choice depends on extent/volume, otoneurological status, age, comorbidities, and functional goals. Genetic evaluation (SDHx) and advanced imaging support stratification and follow-up.
