A 59-year-old patient was referred to an endodontist for retreatment and dental treatment. She also reported emergency treatment on tooth 21. Radiographically, a cast metal post and filling material were observed at the appropriate apical limit, but there were painful symptoms in tooth 11. The crown was removed with a transmetal bur and the metal core was removed using an operating microscope combined with two ultrasounds with combined power. After anesthesia, the coronal opening and gutta-percha removal were performed with the Logic RT 25.08 system, followed by electronic odontometry. The irrigating substance used was 2.5% sodium hypochlorite. Preparation was performed with Solla broad #60.03 files, and foraminal patency was achieved with a Solla Collors 16/02 Glidepath file, one millimeter beyond the apical foramen exit. The preparation of teeth 21 and 22 were performed with the same protocol, Solla wide files #60.03. The root canal system was filled using the single cone technique associated with Sealer Plus cement. It is concluded that endodontic retreatment after removal of a cast metal core and treatment of a tooth with pulpitis can be performed in a single session and determine the success of endodontic therapies.
