A 41-year-old female patient was referred to an endodontist for endodontic treatment of tooth 12. During the anamnesis, edema, mobility, pain on vertical and horizontal percussion, and palpation were observed. Radiographically, extensive and diffuse periapical radiolucency was noted. Drainage of the intraoral purulent collection was performed, followed by coronal opening with a 1012 spherical bur. The working length was determined with an apex locator, followed by irrigation with 2.5% sodium hypochlorite. After patency of the foramen with a Wave One Gold Glider reciprocating file, drainage of the purulent collection was performed. Root canal preparation was performed with a WaveOne Gold 40.05 file, and then BIO-C Temp was introduced as intracanal medication for a period of 30 days. The root canal system was obturated using the single cone technique combined with AH-Plus Jet cement. Clinical and radiographic follow-up was performed 6 months after canal obturation. It is concluded that endodontic treatment combined with intracanal medication resulted in clinical and radiographic success, as extensive periapical bone radiolucency was repaired.
