Peripheral Arterial Disease (PAD) represents one of the most prevalent and underdiagnosed manifestations of systemic atherosclerosis, strongly associated with adverse cardiovascular outcomes and progressive functional impairment. This integrative review aimed to synthesize and critically analyze recent evidence on the epidemiological profile, diagnostic strategies, and therapeutic approaches for PAD, emphasizing the implementation of international guidelines. Findings indicate a rising global prevalence, particularly among high-risk populations such as diabetics, smokers, and the elderly. Early diagnosis through ankle-brachial index and vascular imaging remains crucial but underutilized. Therapeutically, high-intensity statins, antiplatelet agents, and PCSK9 inhibitors have shown substantial clinical benefits. Moreover, machine learning–based predictive models are emerging as promising tools for personalized management, although broader clinical validation is still required. It is concluded that the comprehensive management of PAD demands multidisciplinary and individualized strategies focused on early intervention, therapeutic adherence, and evidence-based integration into local healthcare systems.
