Cauda Equina Hemangioblastomas: Diagnosis, Treatment, and Prognosis.

Abstract

Introduction: Cauda equina hemangioblastoma is a rare, benign tumor located in the intradural extramedullary region of the spine, primarily affecting the cauda equina. This condition presents challenges in both diagnosis and treatment due to the proximity of nerve roots. Objective: To perform a systematic review of documented cases of cauda equina hemangioblastomas, with emphasis on diagnosis, surgical treatment, and long-term prognosis. Methodology: Based on a systematic search of studies in PubMed using keywords such as “cauda equina hemangioblastoma,” “diagnosis,” and “treatment.” Ten relevant studies that met the inclusion criteria were included and critically appraised to identify common patterns and therapeutic strategies. Results: Most cases presented in middle-aged women, with tumors located mainly between levels L1 and L5 of the lumbar spine. Low back pain, radiculopathy, and motor weakness were the most common symptoms. Magnetic resonance imaging was the primary diagnostic tool, and surgery mostly consisted of laminectomy and tumor resection. Discussion: The complexity of surgical treatment was highlighted due to the proximity of the nerve roots, and postoperative complications were common. However, the prognosis was generally favorable with adequate follow-up. Conclusions: Surgical treatment remains the primary option for cauda equina hemangioblastomas, although patients should be carefully monitored to avoid neurological complications.
