Introduction: Anorectal surgery presents challenges in managing postoperative pain. Ultrasound-guided pudendal nerve block has gained attention for its ability to provide more precise pain control and reduce common complications, outperforming traditional techniques. Objective: To evaluate the effectiveness of ultrasound-guided pudendal nerve block in anorectal surgeries compared to traditional anesthesia, considering its analgesic efficacy, recovery, and patient satisfaction. Methodology: A systematic review following PRISMA guidelines was conducted, searching PubMed and Cochrane for original studies published between 2020 and 2025 that compared ultrasound-guided pudendal block to other anesthetic techniques in anorectal surgeries, excluding editorials and irrelevant studies. Results: The ultrasound-guided pudendal nerve block demonstrated effectiveness in reducing postoperative pain and opioid consumption, as well as decreasing complications like urinary retention. Patients reported higher satisfaction and shorter hospital stays, suggesting both clinical and economic benefits. Conclusion: This block, being more precise and safer than traditional alternatives, emerges as a promising option in anorectal surgery. The technique not only improves pain management and reduces complications but also optimizes hospital resources, enhancing the quality of care.
