INTRODUCTION: Diabetes Mellitus (DM) is a chronic disease associated with multiple complications and high mortality, representing a significant public health issue in Brazil. This study aimed to analyze mortality trends due to DM in women in Brazil from 2019 to 2023. METHODOLOGY: This is an epidemiological, descriptive, retrospective, and quantitative study based on secondary data extracted from the Mortality Information System (SIM/DATASUS). All records of female deaths with the underlying cause of death attributed to DM (ICD-10: E10–E14) were included. Data were organized by year of occurrence, region of residence, and age group. RESULTS AND DISCUSSION: During the analyzed period, 196,280 deaths of women due to DM were recorded in Brazil, with the highest concentration in the Southeast (39.5%), followed by the Northeast (31.7%). The year 2021 showed the highest number of deaths, coinciding with the peak of the COVID-19 pandemic, suggesting a worsening in the management of chronic diseases during this period. Mortality was predominantly observed in women over 40 years of age, possibly attributed to aging, insulin resistance, prolonged exposure to hyperglycemia, and the presence of comorbidities. Significant regional disparities were also observed, associated with socioeconomic, demographic, and healthcare access differences. FINAL CONSIDERATIONS: Regional inequalities, socioeconomic and hormonal factors, and unhealthy lifestyles were identified as important determinants of the observed outcomes. The results highlight the need for specific, regionally adapted strategies for diabetes control, focusing on expanding healthcare access, promoting healthy habits, and reducing health inequalities.
