Preservação Alveolar com o Uso da Barreira de Polipropileno (Bone Heal): Um Relato de Caso Clínico

Abstract

Alveolar ridge preservation in patients with systemic disorders, such as type II diabetes mellitus, still lacks well-established minimally invasive protocols, especially in cases associated with infectious processes. This case report describes the management of a 52-year-old melanoderm male patient with type II diabetes, diagnosed with a fracture of tooth #14, who underwent tooth extraction followed by alveolar ridge preservation using a non-resorbable polypropylene barrier, without the addition of biomaterials. The procedure was performed without flap advancement, allowing healing by secondary intention to maintain a wide band of keratinized tissue. After the healing period, the guided bone regeneration not only preserved the alveolar ridge dimensions but also promoted vertical bone gain of the previously fractured buccal bone plate. This bone gain allowed implant placement in an ideal three-dimensional position from both esthetic and functional perspectives. The entire treatment was digitally planned, including cone-beam computed tomography and intraoral scanning, and the implant surgery was performed using a surgical guide. This clinical case suggests that the isolated use of non-resorbable barriers may represent a safe, effective, and low-morbidity approach for alveolar ridge preservation in systemic patients, while also promoting the maintenance of peri-implant keratinized tissue.
