Implant in Upper Premolar with Maxillary Sinus Lift Using Summers Technique Associated with Osseodensification and Immediate: A Clinical Case Report

Tooth loss in the posterior maxilla frequently leads to bone resorption and maxillary sinus pneumatization, complicating implant rehabilitation. Minimally invasive approaches, such as the transcrestal sinus floor elevation described by Summers and osseodensification, have been proposed to address these clinical challenges. This case report describes the treatment of a patient with a medical history of sinusitis and controlled anxiety, undergoing medication for neurological conditions use. The patient presented with loss of tooth 15, complaining that the tooth "broke and I want an implant." Clinically, moderate tooth wear without significant facial alterations was observed. A transcrestal sinus lift using Summers' technique combined with osseodensification was chosen, allowing immediate installation of a 4 mm x 8.5 mm implant with high initial torque (approximately 45 Ncm). An immediate provisional crown was placed without direct occlusal contact. A postoperative medication protocol including antibiotics, corticosteroids, anti-inflammatories, probiotics, and chlorhexidine was prescribed. Postoperative follow-up showed satisfactory healing without complications such as infection or sinusitis. The association of Summers' technique with osseodensification and immediate provisionalization is a predictable, minimally invasive, and effective approach for immediate rehabilitation in the posterior maxilla with limited bone height, resulting in high primary stability and patient satisfaction.
