Shock State: Classification and Initial Management Strategies

This study is a critical and analytical narrative review of the research on the main concepts regarding shock states. A review of articles was conducted in the Scientific Electronic Library Online (SciELO) and PubMed databases using the following Health Sciences Descriptors (DeCS): Shock; Strategy; Management. Articles from 2021 to 2024 were selected in English, Portuguese, and Spanish to enhance the relevance and quality of the review. Emergency Medicine, within the field of medical pathology, applies concepts common to Intensive Care Medicine but in a very distinct setting. In this situation, conventional approaches from these specialties are not always suitable. Issues such as the quality of available information, which is often limited or contradictory, and the need to perform rapid interventions may delay care and harm the patient’s prognosis. Proper management of shock is essential to improve the prognosis of patients in critical situations. The classification of shock, from hypovolemic to cardiogenic, distributive, and obstructive, highlights the importance of a personalized approach, considering the underlying causes of each type of shock. Early identification, systematic assessment, and the application of rapid interventions are crucial for reversing tissue hypoperfusion and improving organ perfusion. Additionally, the use of protocols such as ABCDE, fluid resuscitation, and controlling the primary cause are key interventions that, when effectively performed, can save lives and reduce mortality associated with shock.
