TRATAMENTO CIRÚRGICO X CONSERVADOR EM PACIENTES IDOSOS COM COLELITÍASE ASSINTOMÁTICA: O DILEMA DA IDADE

ABSTRACT: The world population has been growing progressively, at the same time as Brazil is aging rapidly and intensely. Cholelithiasis is the most common abdominal surgical pathology in the elderly population and, considering the importance of the topic, a review was carried out with the aim of analyzing the best treatment choice for the elderly population with asymptomatic cholelithiasis and, in case of surgical need, evaluating whether laparoscopic cholecystectomy is superior to conservative treatment in the elderly with cholelithiasis. To this end, a bibliographical review was carried out based on prospective cohort, case-control, cross-sectional studies,
clinical trials and systematic reviews of scientific articles published in Scielo, Pubmed and VHL Virtual Health Library of the Ministry of Health, using the descriptors gallstones, cholelithiasis, aged, general surgery, surgical technique, conservative treatment, surgical procedures, as well as in Portuguese, isolated and combined. Review of the literature indicates that current practice tends to operate on symptomatic patients while asymptomatic patients are generally monitored, except in cases of high risk of complications. With regard to surgical interventions, advances in laparoscopic surgery have improved operative outcomes in elderly patients, providing lower morbidity and faster recovery compared to traditional open surgery. However, the decision to operate on elderly individuals, especially the most fragile, must be taken with caution, considering the greater likelihood of complications and the impact on the quality of life of these patients.
