Abstract
Introduction: Acute myocardial infarction without ST-segment elevation (NSTEMI) presents challenges in managing frail patients, especially the elderly, with multiple comorbidities. The choice between invasive and conservative strategies is complex. Objectives: To review evidence on invasive and conservative strategies for managing frail elderly patients with NSTEMI, considering their impact on clinical outcomes. Methodology: A bibliographic review of studies published between 2018 and 2025, using databases such as PubMed, Scielo, and MedLine. Studies on NSTEMI, frailty, and invasive treatments were included. Results and Discussion: Studies show underutilization of invasive strategies in frail elderly patients due to concerns about risks and prognosis. No clear evidence supports the superiority of one approach over the other for this population. Conclusion: The invasive strategy did not demonstrate significant benefits compared to the conservative approach. More studies are needed to evaluate the impact of frailty and define the best therapeutic approach for this group.
