Impact of Interdisciplinarity in the Approach to Patients with Chronic Diseases in Primary Care

Introduction: The World Health Organization defines chronic conditions as diseases that require ongoing care, encompassing both non-communicable diseases (NCDs) (such as diabetes and hypertension) and communicable diseases (such as HIV) and mental health problems. In Brazil, there is a correlation between an ageing population and an increase in chronic diseases. It is pertinent to consider that Primary Health Care (PHC) plays an essential role in the prevention and management of these conditions where professional interdisciplinary action guarantees patients with CNCDs comprehensive care capable of restoring their state of health and quality of life (QoL) through PHC. Objective: To analyze the impact of interdisciplinarity on the approach to patients with chronic diseases in Primary Health Care. Methodology: This is a literature review of 10 bibliographic sources listed in the BVS (Virtual Health Library), PMB (PubMed Central) and Scientific Electronic Library onLine (SciELO) databases. The study sample included publications between 2019 and 2024, in Portuguese, English and Spanish, with free access and relevance to the study. Exclusion criteria included studies that were out of scope, written in unspecified languages and inaccessible publications. Results: It is important to emphasize the role of PHC teams through interprofessional education in identifying challenges in the care of the elderly as well as the need for integrated therapeutic approaches to achieve improvements in the management of chronic diseases and considerable advances in their QoL. Discussion: The interdisciplinary approach in PHC has been shown to improve the quality of life and clinical outcomes of patients with chronic diseases, promoting more effective care. However, challenges such as patient resistance to lifestyle changes, lack of resources and unequal access to health services make it difficult to fully implement this approach. Final Considerations: Interdisciplinarity in the care of chronic diseases in PHC aims to treat the patient globally, promoting acceptance and improving adherence to treatment.
