TRATAMENTO CIRÚRGICO DE EXTENSO CISTO DENTÍGERO MANDIBULAR EM PACIENTE IDOSO: RELATO DE CASO

Abstract

The dentigerous cyst is an odontogenic pathology that stands out as one of the most common cystic lesions in the oral cavity. In direct association with the crown of an unerupted tooth, it has a significant prevalence, affecting different age groups. Its clinical manifestation often goes unnoticed, which reinforces the importance of early investigation and accurate diagnosis for effective treatment. This article aims to report a case of surgical treatment of an extensive mandibular dentigerous cyst in an elderly patient. A male patient, Caucasian, 62 years old, SAH+, was referred to the Oral and Maxillofacial Surgery and Traumatology service at the University of Pernambuco after finding a radiolucent lesion in the right mandibular body, associated with the crown of the asymptomatic third molar. As a treatment proposal, the installation of a 2.4mm reconstruction plate in the region of the body and right mandibular branch and the excision of the cystic lesion and the right lower third molar, associated with the lesion, were planned for the case, with prototyping and pre- -modeling of the reconstruction plate. This article emphasizes the relevance of early diagnosis and an assertive surgical approach in the treatment of dentigerous cysts, highlighting the importance of integrating clinical evaluation, imaging exams and histopathological analysis for an effective therapeutic approach. Enucleation as a therapeutic modality, combined with the extraction of the unerupted tooth, proved to be efficient in the case in question, protecting the patient's masticatory and aesthetic function.
