Bastien's coronal opening in a maxillary central incisor and a maxillary right lateral incisor - Case reports

Abstract

This study aims to present a clinical case in which a Bastien opening was performed on a maxillary central incisor and a maxillary right lateral incisor (11 and 12) that presented a non-carious lesion on the vestibular side, and subsequent endodontic treatment and provisional restoration of the tooth. A 51-year-old male patient with no history of systemic diseases sought emergency care in a private practice complaining of intense, spontaneous, pulsating pain in tooth 11. The intraoral clinical examination revealed extensive caries on the vestibular surface of teeth 11 and 12. The recommended instrument for root canal preparation was Prodesign Logic. The obturation was performed using the single cone technique associated with BIO-C Sealer cement (Angelus) and the vestibular wall was restored with Opus bulk fill flow (FGM) composite resin. The bioceramic cement was leaked out in Figures 3 and 4. It is concluded that treatment with modified access using the Bastien technique, performed on upper incisors with caries on the vestibular surface, preserves healthy coronal dentin, determining clinical and radiographic success, which may represent an advantage in relation to dental longevity.
