Abstract

The reduction of alveolar ridge is a progressive condition of multifactorial and irreversible origin that affects many people. it is essential for the success of rehabilitation with implants a correct planning with an appropriate surgical technique for each case.  The present systematic review evaluated whether the performance of a surgical technique with bone grafting influences the survival of implants installed in atrophic maxillae. The systematic review of the literature was developed following the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses PRISMA, with modifications, and the research question was elaborated according to the "peaks" strategy: population, intervention, comparison, outcome and types of studies.

Between 2017 and 2022, a total of 315 abstracts in Portuguese and English were identified in the Medline databases via Pubmed, BBO and Scielo. After selection and evaluation, 11 full articles (primary studies) - randomized clinical trials and clinical case reports - were selected, compared and data extraction performed by two independent reviewers.  Among the surgical techniques applied to gain bone height in implant rehabilitation in atrophic maxillae (moderate to severe) 39% of the studies chose to apply a surgical technique with bone grafting and 61% without bone grafting. Both techniques were associated with a 90-100% implant survival rate with little or no post-surgical complications. It is concluded that the occurrence of failures is small and that the use of bone grafts does not tend to increase implant survival.
