Abstract

This review article addresses modern anticoagulant therapies for the management of atrial fibrillation (AF), focusing on the comparison between non-vitamin K antagonist oral anticoagulants (NOACs) and vitamin K antagonists (VKAs), as well as exploring their applications in specific populations such as the elderly and patients with renal impairment. The review also discusses anticoagulation management in patients with AF and coronary artery disease, highlighting strategies for combining NOACs with antiplatelet agents to reduce bleeding risk. The methodology included a comprehensive search in databases such as PubMed, Embase, and Cochrane Library, with rigorous inclusion and exclusion criteria to select the most relevant and recent studies. The results indicate that NOACs offer comparable efficacy to VKAs in preventing thromboembolic events, with superior safety, particularly in reducing intracranial hemorrhages. Additionally, NOACs have proven particularly advantageous in vulnerable populations, providing a safer and more practical alternative. The discussion emphasizes the need to individualize treatment based on the specific risks of each patient and suggests that the combination of NOACs with antiplatelet agents is a promising approach in patients with AF and coronary artery disease. The conclusion is that NOACs represent a significant advancement in the management of AF, offering an effective and safe therapeutic option, especially in high-risk patients. Future studies are needed to explore the use of NOACs in underrepresented populations, such as those with severe renal impairment, and to further refine clinical guidelines.
