Early Implant Placement Following Atraumatic Single-tooth Extraction in the Esthetic Zone: Case Report

Abstract

The dental rehabilitation of a patient following the extraction of a single tooth in an esthetic area is clinically challenging. One treatment option is the placement of an implant followed by the installation of an implant-supported crown. However, immediate placement is not always possible, and in such cases, early implant placement should be considered due to its high success rate and tissue preservation, which benefits the esthetic area. Therefore, the objective of this study is to report a clinical case of early single implant placement following atraumatic extraction in an esthetic area. A 53-year-old female patient presented to the dental clinic complaining of pain in tooth 22. The initial suspicion was a fracture, which was confirmed after a tomographic examination. The first step was the atraumatic extraction of the left lateral incisor. After 8 weeks, clinical examination revealed uneventful healing of the extraction socket. The implant bed preparation was performed according to the standard protocol, using a sequence of drills with increasing diameter and abundant cooling with saline solution. The implant inserted was a Straumann®️ Bone Level Tapered implant, with a diameter of 3.3 mm and a length of 10 mm, placed in the correct three-dimensional position, indicating good primary stability. Following the placement of the healing abutment, autogenous and xenogenous bone grafting was performed, along with the placement of a collagen membrane using the double-layer technique to enhance stability. After 3 months, the patient was cleared for the installation of a provisional prosthesis and has been under follow-up for 6 months, demonstrating clinically healthy peri-implant soft tissues. In conclusion, in this case, early single implant placement in an esthetic area resulted in excellent healing and surgical success, with tissue health and stability, as well as the preservation of the esthetic region.
