Abstract

Sinus floor communication is characterized as a relatively common morbid condition in dental practice. Although related to several etiological factors, the primary one is the extraction of upper molars due to their anatomical proximity to the maxillary sinus. The aim of this study is to report a clinical case of oroantral communication treatment using the Bichat's fat pad. A 43-year-old male patient, ASA II, sought a private dental clinic due to frequent episodes of inflammation in the posterior region of the maxilla, where he reported having undergone extraction of teeth #16 and #17 six months ago. Given the patient's clinical presentation, Cone-Beam Computed Tomography was requested. Intraoral examination revealed the presence of a fistula in the extraction site, and tomographic examination showed radiopacity in the maxillary sinus, suggesting bone fenestration and sinusitis, respectively. Antibiotic therapy and nasal decongestant were initiated for sinusitis, followed by surgical closure of the oroantral fistula using Bichat's fat pad. Proper management is essential for the therapeutic success of the fistula. This case aligns with the literature, demonstrating that the use of Bichat's fat pad is a convenient and reliable method for reconstructing small to medium-sized intraoral defects.
