Introduction: Burns continue to cause high mortality, primarily due to infections and severe complications. The SAMU 192 and SUS reforms aimed to improve emergency care for burn accidents in Brazilian hospitals. Objective: Assess mortality rates in two cities in the metropolitan area of Vitória, before and after SAMU 192 implementation. Methods: This descriptive and observational study used data on burn cases from Vitória and Serra, Espírito Santo, accessed via DATASUS. Results: Mortality rate significantly decreased from 2.72 to 0.87 after SAMU 192 and SUS reforms. Serra had higher mortality than Vitória, with victims predominantly male. Conclusion: Scald burns were more common and had high morbidity, while burns from open flames had higher mortality. Improved emergency care for burn victims reflects the impact of SAMU 192 and the "Pacto pela Saúde" reforms.
