The gut microbiota, a complex community of microorganisms, has emerged as a crucial modulator of host homeostasis and the response to diseases such as melanoma, a highly aggressive skin cancer. This article reviews the mechanisms by which the microbiota influences the development, progression, and therapeutic response of melanoma. The interaction between the microbiota and the immune system is analyzed, demonstrating how commensal microorganisms modulate innate and adaptive immune responses through interactions with dendritic cells, macrophages, T lymphocytes, and the production of cytokines and chemokines. Additionally, the impact of microbial metabolites, such as short-chain fatty acids, secondary bile acids, and diet-derived phenolic compounds, on cellular signaling and tumor microenvironment modulation is discussed. Emerging strategies, including the use of probiotics, prebiotics, selective antibiotics, fecal microbiota transplantation, and dietary interventions, are evaluated for their potential to optimize therapeutic responses and mitigate adverse effects. The integration of approaches based on microbial modulation could transform melanoma treatment, improving therapeutic efficacy and patient quality of life. Future research should focus on personalizing these interventions, taking into account individual microbial compositions and the underlying mechanisms of these complex interactions.
