Abstract

This article reviews the management of acute liver failure (ALF) in patients with chronic viral hepatitis, a severe condition that results in high mortality and requires prompt and effective interventions. Chronic viral hepatitis, particularly those caused by hepatitis B (HBV) and hepatitis C (HCV) viruses, can progress to ALF due to viral reactivations, superinfections, or factors such as alcohol consumption and the use of hepatotoxic drugs. The aim of this study was to review the existing literature to identify effective strategies for the prevention and clinical management of ALF in patients with chronic viral hepatitis. The methodology involved an integrative review of articles published between 2013 and 2023, using databases such as PubMed, SciELO, and LILACS, with strict inclusion criteria to ensure the relevance of the studies analyzed. The results indicate that ALF in these patients is often precipitated by a combination of factors, including viral reactivations and exposure to hepatotoxic agents. Effective management of ALF requires suppression of viral replication through the use of antivirals, intensive care support, and, in severe cases, emergency liver transplantation. Liver transplantation has been shown to be the most effective intervention for critically ill patients, offering significantly higher long-term survival rates. However, prevention remains the most effective focus, emphasizing regular monitoring of liver function, strict adherence to antiviral therapy, and patient education on the risks of hepatotoxic substances. It is concluded that while liver transplantation is crucial for saving lives in advanced cases, preventive strategies and early interventions are essential to reduce the incidence of ALF and improve clinical outcomes in patients with chronic viral hepatitis.
