Inguinal hernia (IH) is a matter of significant concern among surgeons, primarily due to the high recurrence rate and its complications, as approximately 10% of hernias recur after surgery. Techniques and materials have long been studied to reinforce the abdominal wall in the surgical treatment of hernias. In 1957, Alcino Lázaro da Silva, a student at the Federal University of Minas Gerais (UFMG), began studying the anatomical piece discarded in the surgical block during an incisional hernia correction operation. However, it was only in 1968 that it became possible to treat a patient with the TRANSPALB technique or Lázaro da Silva Technique. This study aims to present the history and evolution of the TRANSPALB Technique, and its application in contemporary incisional hernia surgeries. After analyzing the studies, it is concluded that the use of Lázaro da Silva's technique can currently be applied in situations where there is no mesh and there exists a hernia sac resulting from the hernia
