Abstract

Introduction: Smoking, a leading preventable cause of cardiovascular diseases, persists despite reduced smoking rates. Electronic cigarettes (ECs) emerge as an alternative, yet their growing popularity raises concerns due to lacking data on true health impacts. Transitioning to ECs raises questions about potential health benefits. Methodology: Data were obtained through a systematic literature review using MEDLINE and Scopus from 2019 to February 2024. English studies on ECs' cardiovascular impacts were selected, excluding other organic systems and specific document types. Results and discussion: Research indicates ECs generate a sympathetic response akin to traditional cigarettes, elevating blood pressure and heart rate. Nicotine-containing EC vapor exposure relates to increased thrombogenicity and altered microvascular function. Vascular function changes post-smoker transition to ECs suggest potential benefits, albeit requiring long-term investigations. Epidemiological studies suggest reduced respiratory outcomes in ex-smokers using ECs, with no significant differences in cardiovascular outcomes. EC vapor inhalation may alter vascular markers, indicating adverse effects, but smoking cessation may improve vascular function. Further research is needed to comprehensively evaluate ECs' risks and benefits compared to conventional smoking cessation methods. Conclusion: Inhalation of EC vapor, especially with nicotine, increases sympathetic nervous activity, blood pressure, and heart rate. While it may improve vascular function short-term, long-term effects on cardiovascular health remain uncertain. Smoking cessation remains the best strategy.
