Palliative sedation in terminal cancer patients in palliative care: a literature review

Abstract

This article aims to evaluate the clinical aspects of palliative sedation performed in the last 4 years in oncological patients, taking into account the prevalence, indication, type of sedation, and the drug used. Integrative review in the databases of BVS, LILACS, SciELO, PubMed of works published between 2019 and 2023, combining the descriptors "palliative care," "sedation," and "cancer" with the Boolean descriptor "AND." The inclusion criteria were articles published in English or Portuguese, adult patients, carriers of neoplasia in palliative care, and who were subjected to palliative sedation. Out of 41 articles, 5 were included. The most frequent indications for palliative sedation were delirium, dyspnea, and pain. Other symptoms present in the studies were nausea, vomiting, and existential suffering. Benzodiazepines, mainly midazolam, were the most used drugs both in continuous and intermittent sedation. It is concluded that palliative sedation is an important comfort therapy for end-of-life patients. Further research should be conducted to consider other therapeutic options in palliative sedation.
