REPAIR OF PERIAPICAL LESION OF ENDODONTIC ORIGIN: REPAIR OF PERIAPICAL LESION

Abstract

This work aims to present a clinical case of retreatment with the regression of an endodontic lesion, which simulated an endoperium lesion, with monthly changes of calcium hydroxide paste (UltraCal®) and continued repair of the lesion. Female patient, 47 years old, was referred for dental treatment due to the need for retreatment in element 46. On clinical examination, the presence of a fistula was observed and the patient reported pain on vertical and horizontal percussion, there was an absence of periodontal pocket and the X-ray examination revealed extensive periapical bone radiolucency also involving the furcation region and the presence of filling material, as well as apical external resorption. Retreatment was performed using rotary instrumentation (Prodesign Logic RT ®). 2.5% Sodium Hypochlorite was used as the irrigating solution, and the intracanal medication used was calcium hydroxide paste (UltraCal®) and the crown was sealed with glass ionomer cement. The calcium hydroxide paste was renewed monthly in order to observe the reduction of the lesion. When there was regression of more than half of the lesion, the root canal system was filled. It is concluded that endodontic retreatment associated with renewal of intracanal medication determined clinical and radiographic success, ruling out the need for periodontal therapy.
