Abstract
Although the Integrated Health System (SUS) has achieved many things, the rapid aging of the population, the changing nature of the epidemic and many socioeconomic changes have, in recent years, caused new challenges for the SUS. Among them, care for patients with chronic illnesses and problems that arise in the existing health system, at the macro level (administrative, legal, autonomous) and at the micro level, that is, those involved in: work processes. Data collection was carried out through the following: Nursing Database (BDENF), Online Scientific Electronic Library (SCIELO), PubMed and Literature in Health Sciences of Latin America and the Caribbean (LILACS). In this sense of appropriate group care, this article will focus on the process of creating a local health network; government work; strengthening primary health care (PHC); the role of PHC in the treatment of critically ill patients. In this sense, we will describe and comment on recent actions by the Ministry of Health that are equivalent to the concept of treating people with chronic diseases.
