O processo supracondilar do úmero é uma variação anatômica que ocorre na parte ântero-medial do úmero, distando de 5 a 7 centímetros do epicôndilo medial superiormente. Nessa estrutura fixa-se, ocasionalmente, um ligamento chamado Ligamento de Struthers (LS), na parte proximal do processo supracondilar e no epicôndilo medial em sua parte distal. Essa variação anatômica é dita por alguns autores como uma anomalia congênita rara, já que pode comprimir estruturas importantes que passam próximas a essa variação, como as artérias e veias braquiais e os nervos mediano e ulnar. Esse processo ocorre a partir da fixação de fibras anômalas do músculo pronador redondo que estão presentes quando há essa variação anatômica, fazendo com que, quando o cotovelo é estendido, haja a compressão neurovascular. Os objetivos deste artigo são apresentar e discutir a importância em reconhecer o processo supracondilar para o correto tratamento em casos de compressão de nervos ou vasos, bem como o diagnóstico diferencial, através de revisão literária e coleta de dados, além de contribuir com mais um relato de caso sobre tal variação anatômica tão importante. Uma das possibilidades de correção desse tipo de obstrução é a cirurgia, sendo essa responsável por alta taxa de sucesso na correção do problema.
