Gestão da Insuficiência Hepática Aguda em Pacientes com Hepatite Viral Crônica

Resumo

Este artigo revisa a gestão da insuficiência hepática aguda (IHA) em pacientes com hepatite viral crônica, uma condição grave que resulta em alta mortalidade e requer intervenções rápidas e eficazes. A hepatite viral crônica, especialmente as causadas pelos vírus da hepatite B (HBV) e C (HCV), pode evoluir para IHA devido a reativações virais, superinfecções ou fatores como a ingestão de álcool e uso de medicamentos hepatotóxicos. O objetivo deste estudo foi revisar a literatura existente para identificar estratégias eficazes de prevenção e manejo clínico da IHA em pacientes com hepatite viral crônica. A metodologia envolveu uma revisão integrativa de artigos publicados entre 2013 e 2023, utilizando bases de dados como PubMed, SciELO, e LILACS, com critérios de inclusão rigorosos para garantir a relevância dos estudos analisados. Os resultados indicam que a IHA nesses pacientes é frequentemente precipitada por uma combinação de fatores, incluindo reativações virais e exposição a agentes hepatotóxicos. O manejo eficaz da IHA requer a supressão da replicação viral através do uso de antivirais, suporte intensivo em unidades de terapia intensiva, e, em casos graves, o transplante hepático emergencial. O transplante hepático mostrou-se a intervenção mais eficaz para pacientes em estado crítico, proporcionando taxas de sobrevivência significativamente maiores a longo prazo. No entanto, a prevenção continua sendo o foco mais eficaz, com ênfase no monitoramento regular da função hepática, adesão estrita à terapia antiviral, e educação dos pacientes sobre os riscos de substâncias hepatotóxicas. Conclui-se que, embora o transplante hepático seja crucial para salvar vidas em casos avançados, estratégias preventivas e intervenções precoces são essenciais para reduzir a incidência de IHA e melhorar os resultados clínicos em pacientes com hepatite viral crônica.
