Alopecia no cicatricial durante el tratamiento con tirzepatida: Revisión sistemática.

Resumo

Introducción: La pérdida de cabello ha sido descrita durante el tratamiento con tirzepatida, aunque persisten dudas sobre su frecuencia, presentación clínica y relación con la reducción ponderal. Objetivo: Evaluar la evidencia disponible sobre alopecia no cicatricial durante el tratamiento con tirzepatida e identificar los factores asociados con su aparición. Métodos: Se realizó una revisión sistemática en PubMed/MEDLINE hasta el 30 de julio de 2026, siguiendo PRISMA 2020. Se incluyeron ensayos clínicos, estudios observacionales, transversales y análisis de farmacovigilancia con datos específicos de tirzepatida. Resultados:     Se incluyeron 20 publicaciones correspondientes a 19 estudios independientes: seis ensayos clínicos, siete estudios observacionales o transversales y seis análisis de farmacovigilancia. En los ensayos controlados con placebo sin exposición previa, la alopecia ocurrió en 2,9–7,0% de los participantes tratados con tirzepatida frente a 0,6–1,4% con placebo. Los estudios observacionales identificaron asociaciones con efluvio telógeno y alopecia androgenética. Una mayor pérdida ponderal y el sexo femenino fueron factores asociados en estudios transversales. Las señales de farmacovigilancia fueron heterogéneas. Conclusiones: La pérdida de cabello puede presentarse durante el tratamiento con tirzepatida, pero la evidencia actual no permite establecer un mecanismo causal único. La reducción ponderal podría contribuir al fenómeno.
